Responsibilities:
- Provide skilled nursing care to patients.
- Perform patient assessments and develop treatment plans.
- Administer medications and treatments as prescribed.
- Collaborate with the interdisciplinary team to ensure comprehensive patient care.
- Communicate effectively with patients, providers, and other team members.
- Provide case management and hospice care services.
Required Skills & Certifications:
- Clinical Assessment, Medication Administration, and Treatment Planning skills
- Minimum of two years of hospital, home health, or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of working
- Bachelor's degree in Nursing or related field
- Driver's license, car insurance, and automobile for field work

Scheduling:
- 8-hour day shifts, primarily 7:30 am - 3:30 pm with some 11:00 am - 7:30 pm weekday shifts.
- 40 hours per week.
- Weekend requirement: Every 2nd to every 3rd weekend (7:00 am - 3:30 pm).
