We are a multispecialty practice encompassing Family Medicine, Pediatrics, OBGYN and clinics. The center was established in 1974 in a brownstone house that was donated by Dr. Edward Osborn, the first Chief of Surgery at Our Lady of Lourdes Medical Center. For over 30 years, Osborn Family Medical Centers, has provided primary healthcare services to local communities, including residents and undocumented immigrants. We are looking for a talented Family Medicine Nurse Practitioner, to deliver Comprehensive Primary Care Services to our population. Our team of providers are passionate, culturally sensitive in providing quality of care to our patients while achieving healthy outcomes. JOB SUMMARY: The NP will independently assess, diagnose, prescribe medications and treat patients according to the established protocols in collaboration with physicians. The NP will function within the scope of practice as defined by the state nurses practice act, delineation of privileges set forth by the practice setting and the American Nurses' Association Code of Ethics. Provide triage referral coordination, develops differential diagnosis, and individualized care plans along with patient education for specific diseased types as appropriate.
