Job Description

Description:

Advanced Practice Provider (APP) – Primary Care with Hormone Therapy Experience
Shape Your Career. Expand Your Impact. Own Your Future. Ascend Medical is redefining primary care — and we want Advanced Practice Providers (Nurse Practitioners and Physician Assistants) who are ready to g row, innovate, and take control of their future. This role allows you to practice alongside top clinical leaders in a patient-centered, whole-person care model — one that integrates physical health, mental health, metabolic wellness, and hormone balance. We’re especially seeking an APP comfortable treating women across all stages of life — with experience evaluating, managing, and optimizing hormone replacement therapy (HRT) as part of comprehensive primary care. In this role, you’ll have the freedom to shape your career while delivering exceptional, personalized care to a diverse patient population. We offer a highly competitive base salary with significant bonus opportunities and employee stock options, making you not just part of the team — but an owner in our success.
Company Summary Founded in 2021, Ascend Medical’s mission is simple yet profound: to elevate body, mind, and soul. Our advanced primary care model integrates behavioral health, metabolic health, hormone health, and emotional well-being to keep patients thriving physically, mentally, and emotionally. Our Values

  • Patients before Profits
  • Empathy before Expedience
  • Progress before Stagnation
  • Team before Individual
  • Safety before Everything

At Ascend Medical, you’ll join a team of compassionate innovators who are forging a new path in healthcare — one that is patient-centered, accessible, and deeply personal. With a Net Promoter Score consistently above 95 , Ascend leads the industry in patient satisfaction and delight.
What You Can Expect in This Role

  • Provide comprehensive, integrative care across multiple settings — in-office and via telehealth
  • Support women through perimenopause, menopause, and related hormonal transitions by integrating HRT management into preventive and lifestyle-based care.
  • Evaluate, diagnose, and treat patients following evidence-based care pathways, including management of chronic conditions, preventive care, and hormone optimization.
  • Collaborate with physicians and specialists on complex cases to ensure holistic treatment plans.
  • Guide patients through their wellness journey, educating them on lifestyle, nutrition, hormone balance, and preventive health strategies.
  • Communicate proactively and compassionately with patients to deliver an exceptional experience.
  • Conduct health risk assessments, manage ongoing treatment plans, and ensure continuity of care.
  • Leverage technology and integrated EMR systems to streamline workflows and maintain documentation accuracy.

Compensation & Benefits

  • Base salary: Starting base of $140,000 (full-time) + significant quarterly bonus opportunities
  • Equity: Employee stock options — share in the company’s success
  • Benefits: Medical, dental, and vision coverage starting day one
  • Retirement: 401(k) with company match
  • Well-Being: Generous PTO, paid parental leave, and CME stipend
  • Career Development: Ongoing growth opportunities in clinical innovation, HRT specialization, and leadership
  • Culture: Innovative, mission-driven, and centered on holistic care

  • 3+ years of clinical experience, preferably in primary care, endocrinology, GYN, or restorative health
  • Demonstrated experience in hormone therapy (bioidentical hormone replacement, anti-aging medicine, functional medicine)
  • Master’s degree from an accredited NP or PA program
  • Active, unrestricted Georgia NP or PA license
